Dr. Sam’s 1:1 Source Code Coding Container Waiver


I know that this coding container is not therapy, nor is it a substitute for therapy. I am responsible for my own mental health and if I am in need of any professional or medical help, I will seek it out.

I understand that Dr. Sam is not my therapist, she will not be doing any type of clinical psychology with me, nor using any psychology practices during our work together. This is a coding relationship only.

I understand that Dr. Sam will be using the technique of Source Code, which includes focusing on my Coping Styles and helping me live into their Antidotes.

I understand that this work may be emotionally stimulating, and I will not hold Dr. Sam Rader or any of her businesses responsible for my emotional wellbeing.

I understand Dr Sam promises to hold confidential any information I share with her in the coding sessions.

I understand that what I put into this coding program is what I will get out of it. My personal growth is my own responsibility.

I understand that since the scheduling of an appointment involves the reservation of time specifically for me, I will not reschedule appointments unless I am on vacation or there is a true emergency. Any changes in appointment time must be made within 48 hours of the scheduled appointment, or I will forfeit my coding session.
